The RPS cover for wall-mount of the Acc Kits in the BOM was not correct; it should be 22P instead of 14P. The issue is a safety issue for those who choose to wall-mount the RPS. It's found on WK48P/NZ48P that the magjack connector's shielding were short to the test via 2.5v to ground. Adding capton tape underneath Belfuse/Pulse connector to avoid the short via ECO as a short term solution will respin PWB to correct issue.

MGX-RPM-XF-512 boards shipped after Dec 1, 2006 use a specific version of SRAM which may be affected by power supply noise. This can cause the SAR Controller internal memory register to corrupt and cause a board crash.

This software issue may affect customers using MGCP and CAS signaling feature on MGX VXSM board. Service Providers use CAS signaling feature for various call services. In rare circumstances, the software goes into a "race condition" between call process messaging and call is not getting cleared properly. The suspect circuit may appear in a hung state on the VXSM card. Once in a hung state, no future calls will succeed on the suspect circuit. This is a software issue - please do not RMA the VXSM board as the board is working properly.

In the two known instances of mis-wiring, a reversal of the "live" and "ground" conductors in the appliance plug of the power cord set was found. If a mis-wired power cord set is connected to the power source, the chassis can become "live". Any contact with the chassis after it is connected to the power source could result in an electric shock.

The Catalyst 4500 E-Series chassis such as WS-C4503-E and WS-C4506-E will only boot with minimum IOS version 12.2(37)SG, while the Catalyst 4500 E-Series chassis such as WS-C4507R-E and WS-C4510R-E will only boot with minimum IOS version 12.2(40)SG. Besides the minimum IOS mentioned above, E-chassis support was also ported to the Maintenance release 12.2(31)SGA6 and higher for Supervisors other than WS-X45-SUP6-E to resolve the issue.

The IGX8400 supports 4 cell buses for passing data. Most of the cards in the IGX8400 use Cell Bus 1, exclusively, for passing data. Some cards, such as the IGX-URM, IGX-UXM and IGX-UXME use additional cell lane buses two, three and four. In addition to Cell Buses one through four, there is a fifth cell bus, Cell Bus R, used for redundancy when Cell Bus one fails. Cell Buses two through four are not protected by Cell Bus R. Besides the cell buses, the IGX8400 also has control and clock buses named A and B. In this irregularity, the Universal Router Module (URM) connections fail when using Bus B. The loss of the B Bus control and clocking causes data to fail to pass to cell buses.

The original boot flash memory size on Catalyst 8540 route processors (RPs) was 8MB. Currently released Campus Switch Router (CSR) images, including CSR release 12.0(4a)W5(11a) and 12.0(5)W5(13) fit into the 8MB boot flash single in-line memory module (SIMM). However, the next feature release of CSR images is too large to fit into the 8MB boot flash SIMM. This CSR release is currently slated for the second quarter of 2000.

The MPSM-16-T1E1 metal EMI gasket failure was determined to be caused by a suspect gasket rework process at a downstream supplier. Due to the suspect rework process, the gasket may lose adhesion and come off when inserted or removed from the chassis. To date, less than 1% of the field population has experienced this failure. All new versions of the product (800-22339-11 A0 and later) correct the problem by incorporating a faceplate assembly with a new EMI gasket which is applied by the original faceplate supplier.
