Field Notice: FN - 63372 - MCS-7825-I4, Windows 2003.1.5 fresh install fails to build the RAID array, Upgrade required
When Cisco's modified Windows 2003.1.5 is fresh installed on any MCS-7825-I4 or equivalent server, the RAID array may not be created properly. At any point after installation, if a hard drive fails, the non-functional RAID array will not compensate for the hard drive failure. The result is that a loss of service is likely to occur when a hard drive fails. This affects both MCS servers and IBM equivalent servers, as seen in the Products Affected section. Field Notice: FN - 63410 - Cisco Agent Desktop (CAD) for UCCE 8.0(1) unable to login
In the CAD 8.0(1) release, the CAD LDAP service is susceptible to glue nodes, which corrupt the directory and can result in one or more agents being unable to log into the ACD. In addition, a CAD LDAP service with glue nodes cannot be backed up and restored, thus impairing upgrades to later CAD releases. Glue nodes are relatively rare occurrences, but the problem has been reported.

Field Notice: FN - 63251 - WS-C3560V2-XXXX-X, WS-C3750V2-XXXX-X Series RPS cover and rare power reset - Replacement Recommended
The RPS cover for wall-mount of the Acc Kits in the BOM was not correct; it should be 22P instead of 14P. The issue is a safety issue for those who choose to wall-mount the RPS. It's found on WK48P/NZ48P that the magjack connector's shielding were short to the test via 2.5v to ground. Adding capton tape underneath Belfuse/Pulse connector to avoid the short via ECO as a short term solution will respin PWB to correct issue.

A limited number of HP DL380-G6 servers shipped by HP prior to November 16, 2009 and configured with a system board revision earlier than revision "0S," may unexpectedly reboot. A user will see "Server Reset, Server Power Removed, Server Power Restored" message in the Integrated Lights-Out 2 (iLO2) Event Log. A user can determine if they have an affected server by looking at the serial number. It will be xxx947xxxx or earlier.

Field Notice: FN - 61703 - NPE-G1, uBR7200-NPE-G1, C7301 and 7304-NPE-G100 May Experience a Software Error Resulting in an Unexpected Reload Due to Watchdog Timeout (WDT)
The router has a watchdog system on board which generates interrupts every four milliseconds. The software responds to the interrupts and resets the watchdog. If the software fails to respond and resets the watchdog within 32 milliseconds, the watchdog will first issue a Non-Maskable Interrupt (NMI) to the CPU. If the CPU fails to respond to the NMI a further 42 milliseconds, the watchdog will then issue a "L2 watchdog timeout" and cold reset the router. Under certain rare conditions, specific code sequences can prevent the CPU from executing additional instructions. New versions of software are available which eliminate these failures without affecting system performance or functionality. June 23, 2005: Two DDTS caveats have been added to the DDTS list.

An internal failure of certain hard drives with firmware prior to version 3B06 used inside the servers that are listed in the Products Affected section can lead to an issue where the file system on the hard drive is marked 'Read-Only', preventing the software from being able to execute write operations.

Field Notice: FN - 63291 - Cisco Computer Telephony Integration Object Server (CTIOS) CTIOS Server Assertions
During high call volume in production, CTIOS may restart or assert with a minidump, failure to accept new client connections due to previous exceptions that may cause agent login failure, button enablement issues related agent state or call control. If CTIOS Server node did not restart automatically, it may need to be cycled via Cisco ICM Service Control to recover from this condition. Note: We have observed the problem to be more frequent, exposed under load with variable degree of agent counts and call volumes depending on customer environment and call flows.

Field Notice: FN - 62767 - Cisco Unified Contact Center Express (Unified CCX) - Considerations for 2009 US Daylight Saving Time Change
On August 8, 2005, the Energy Policy Act of 2005 (H.R.6.ENR), was signed into law. Section 110 of this Act modified the time change dates for Daylight Saving Time (DST) in the U.S. Beginning in March of 2008, DST will begin on the second Sunday of March and end the first Sunday of November. For 2008 and beyond, the daylight saving time period will be: 2:00 A.M. on the second Sunday in March to 2:00 A.M. on the first Sunday in November.

Field Notice: FN - 62452 - PA-8B-S/T - Line Protocol Failure or System Crash - RMA on Failure
Various failures have been reported on one (1) specific TAN and PCA version (73-2194-04, 800-02060-04) of the PA-8B-S/T port adaptor. These failures can range from ISDN line protocol failures to a system crash. It has been determined that the field programmable gate array (FPGA) firmware resident on the affected PA-8B-S/T port adaptors has a defect resulting in an invalid PCI cycle. This issue will require replacement of the affected port adaptor for a replacement unit with updated FPGA firmware. PA-8B-S/T FPGA code is not field upgradeable.

Field Notice: FN - 62533 - Safety Notice - Potential Manufacturing Defect on Power Cord
In the two known instances of mis-wiring, a reversal of the "live" and "ground" conductors in the appliance plug of the power cord set was found. If a mis-wired power cord set is connected to the power source, the chassis can become "live". Any contact with the chassis after it is connected to the power source could result in an electric shock.

Field Notice: FN - 63255 - MCS-7815-I2 and MCS-7825-I2, BIOS error prevents a system from booting if rebooted after October 1, 2009
This is caused by a defect in the IBM BIOS and is documented in IBM RETAIN Tip: H182636 The System BIOS for xSeries 206m and xSeries 306m contains the on-board HostRAID BIOS. BIOS version 1.42 and 1.43 were integrated with HostRAID BIOS 2106. The HostRAID BIOS 2106 had a calendar limit flag set for the usage. It was compiled with the calendar flag set in error.

Certain Seagate hard drive models used in HP servers (listed in the Products Affected section of this document) that are configured with firmware versions prior to HPG6 might experience drive failure indications. Failures might occur when the disk drives appear to be idle or when online and active. Any hard drive listed in the Products Affected section abovewith a firmware version prior to HPG6 must be upgraded to HPG6 or later to avoid service impacting problems.

Field Notice: FN - 63243 - Microsoft Security patch MS09-014 causing IE 6.0 to crash while generating WebView reports and collecting logs for Support Tools
When you choose any drop-down menus, this action appears to trigger the IE crash, which includes the Date or Month that uses the Calendar for WebView reports and the "Relative Time Span" drop-down menu. The use of Support Tools log collection also brings down Internet Explorer 6.0 with Microsoft security update MS09-014 applied. MS09-014 Addresses a vulnerability in Microsoft Explorer (KB 963027).

Field Notice: FN - 62535 - NPE-G2, Incompatibility With Lower-Revision VXR Series Chassis With Specific Port Adaptors - RMA required
The above noted revisions of the NPE-G2 have been found to be incompatible with earlier revision Cisco 7204VXR and 7206VXR chassis when used in conjunction with the specific port adaptors noted above. The affected NPE-G2 processors were manufactured with a PCI bridge component that is not compatible with the PCI bridge component used in the earlier revision 7204VXR and 7206VXR chassis. The PCI bridge component used in the affected chassis has been found to manipulate PCI data transactions to the PCI bridge on the NPE-G2 in such a way that the data transactions can become corrupted. This data corruption will lead to a failure of the port adaptor and a possible system crash. This issue mostly affects the noted revisions of the NPE-G2 processor when installed in 7200 VXR series chassis manufactured under the above noted top assembly numbers (TAN) and hardware revision number ranges (2.7 and below). A limited number of chassis with TAN and hardware revisions above 2.7 are also affected. An IOS command is available to verify if the suspect chassis is affected with this issue. Most of the affected chassis were shipped prior to (approximately) February 2004.

Field Notice: FN - 62514 - C7200-JC-PA - Certain Jacket Cards with PA installed on 7200VXR may have infrequent system crashes due to PCI Bus Error, Software Forced Crash, WDT Reset error - RMA required
System crashes have been observed on 7200VXR systems when the PA is installed in a C7200-JC-PA port adaptor jacket card. The system crashes could occur when a bidirectional traffic stream is frequently started and stopped. The following errors: PCI Bus Errors, Software Forced Crash errors, and WDT errors are only seen at the onset of traffic. Note: Port adaptors can be installed with or without jacket cards. Only port adaptors installed in jacket cards are affected by this issue. Types of PAs supported with C7200-JC-PA on 7200VXR.

Field Notice: FN - 62725 - 7300-NSE-100, 7304-NPE-G100, C7200-I/O, C7200-I/O-2FE/E, C7200-I/O-GE E, CISCO7301, NPE-G2, NPE-G1, uBR7200-NPE-G1 - Unable To Read CF or PCMCIA ATA Flash Disk - Data Loss or System Boot Hang - Replace Flash Card - RMA Required
This issue affects a specific subset of the PCMCIA ATA Disks and Compact Flash cards that were installed in specific Cisco routers manufactured between May 2005 and October 2006. Cards shipped individually as spares over that same time span may also contain one of the affected units. This issue has been linked to a component manufacturing problem. The root cause was identified and appropriate steps taken to resolve the problem. Product impact varies due to different flash memory access methodologies and use patterns. Some products may be able to mitigate the potential effect by re-writing affected memory areas at specific intervals. This feature does not work with all products, so please check the workaround section for availability and specific instructions.

An MCS Server listed in the Products Affected section of this field notice with any of the HP Ultra320 SCSI hard drives listed in HP Customer Advisory c00677430 may exhibit reduced performance and/or excessive timeouts. This is caused by the dynamically adjusted seek time profile table in the drive firmware, after it becomes degraded.

The Catalyst 4500 E-Series chassis such as WS-C4503-E and WS-C4506-E will only boot with minimum IOS version 12.2(37)SG, while the Catalyst 4500 E-Series chassis such as WS-C4507R-E and WS-C4510R-E will only boot with minimum IOS version 12.2(40)SG. Besides the minimum IOS mentioned above, E-chassis support was also ported to the Maintenance release 12.2(31)SGA6 and higher for Supervisors other than WS-X45-SUP6-E to resolve the issue.
